Couple of things to consider:
1. IF there is a bug, maybe it's in the failure to notify if the target is guarded?
2. Doesn't a ward decrease the chance of success by a fixed percentage? I.E. if a level 20 agent tries to kidnap an envoy (I'm assuming a 100% success rate) who is warded, wouldn't his chance by more along the lines of 80% or even 60% (don't know what the ward contributes). And that's not including the % hit that Cipher mentioned in an earlier post regarding escape.
Might be totally of base, but food for thought.
I've made a correction to the formula and posted a note in known issues as well.

Now if someone would grab data for HP's dying on artifact location divinations. That seems to be the only time they die on me
